Does anyone have an original Pre-Delivery Service Sheet filled out?
Here is an example of the sheet, but it is blank. (sorry about the pour lighting...the flash screwed up the lighting).

Over the years I have seen originals typed and also done in cursive . All of the original ones I have seen used a simple check in the boxes.Just what I have seen .Others may have seen different.
