I believe I got mine from Jim Cowles (Shelby Parts and Restoration in Green Bay) several years ago.

If you have the logo go to a shirt shop or send them the logo. Place the logo or two or 4 of them on the shirt and let them print it for you. Got my shirt with two logos for a tener. Then cut the shirt and voila you have labels. :)
